Preparing the request does not give someone permission to decide for your child. The required guardians still need to agree.

If you need another consent later, you can reuse the details from a previous document to start a new draft. The new consent needs its own purchase or an available credit for that form.

Yes—check the requirements for your departure, any countries you pass through, your destination and your airline or other travel provider. They may ask for their own form, an original signature, notarisation or other documents.
